Madness Penalties question

Just a sanity check , as an Insanity occurred recently in an online game over Fantasy Grounds 2 I was running, it made me revisit the rules, and I got a tad confused.

You get -1 penalty for each Madness level, just like physical wounds, until you receive the fourth one, and gain an Insanity. After this, do you
(a) revert to zero Madness level and start again, or
(b)do you stay at -3 penalties, and gain a new Madness each time you fail, or,
(c)something else ?

I think it it is (a) but thought best to check, as I haven't found this clearly described in the rules. If it is there, please tell me the reference page

It would be either B or you can have them pass out/faint (as per incap...they are out a d6 hours and lose a point of Sanity permanently and awaken still at -3 Madness).
